Who is responsible?
Glen Mitchell, trading as GM Therapy in Wolston, CV8 3LJ, is the controller of personal information collected through this website. Contact glen.mitchell@gmtherapy.co.uk or call 07877 168527 with any privacy question or request.
What the enquiry form collects
The form asks for your name, email address, the session or programme you are interested in, and a brief message. It also records whether you explicitly consent to the use of any health information you choose to include, the time of submission and the version of this notice. Name, email and a message are needed to send an enquiry; selecting a particular service and giving health-information consent are optional.
Please do not include medical history or detailed sensitive information. If you voluntarily include health information, please use the separate consent checkbox. You can instead leave that information out and ask to discuss it privately.
Why your information is used
Glen uses enquiry information to respond, answer questions and discuss possible appointments. Where you ask about booking or receiving a service, the lawful basis is taking steps at your request before entering a contract. For other enquiries, the basis is GM Therapy’s legitimate interest in responding to correspondence and running the practice.
If you choose to provide health information and tick the consent box, its use to respond to your enquiry relies on your explicit consent. You can withdraw this consent by contacting Glen. Withdrawal does not affect processing already carried out lawfully. Without that consent, the form should be used for general, non-health information only.
Enquiries are not used to subscribe you to marketing. No automated decisions or profiling are carried out by the enquiry form.
How messages reach Glen
When the live form is enabled, submissions are processed by Cloudflare, the website hosting and email-delivery provider, and sent to glen.mitchell@gmtherapy.co.uk. Microsoft 365 processes and stores the email in the practice mailbox. The website does not create a separate database of enquiries or save form drafts in your browser. Information remains in the form until it is successfully submitted, you leave the page or you clear it.
Cloudflare processes technical information such as your IP address, request time and browser or network details to deliver and protect the website. The form uses your IP address to limit repeated submissions and a hidden field to help identify automated submissions. The application does not log message contents. Infrastructure and email providers may retain operational records under their service terms.
The Freedom Formula affiliation does not automatically give Freedom Formula access to website enquiries. Information may also be disclosed where required by law.
International processing
Cloudflare operates internationally, so information may be processed outside the UK. Its Data Processing Addendum describes its transfer safeguards, including the UK transfer addendum where applicable. Microsoft 365 may also process information internationally under its applicable data protection terms. Contact Glen for information about the providers and safeguards applicable to your enquiry.
How long information is kept
Enquiries are kept for as long as needed to respond and deal with relevant follow-up. In deciding when to delete them, Glen considers whether the enquiry is resolved, whether an appointment is being arranged, and whether the information is needed to handle a complaint or meet a legal obligation. Information needed for ongoing therapy becomes subject to the separate client privacy information. Provider delivery records and backups follow the relevant provider’s retention arrangements.
Your rights
Depending on the circumstances, you can ask to access, correct or erase your information, restrict its use, or receive portable copies of information you supplied. You may withdraw consent for health information at any time.
You have the right to object to processing based on legitimate interests. Contact Glen using the details above to exercise this or another right. These rights are subject to the conditions and exceptions in data protection law.
You can also complain to the Information Commissioner’s Office (ICO).
Cookies, therapy records and WhatsApp
The website code does not set analytics, advertising or tracking cookies. Any security cookies introduced by hosting settings will need to be reflected in this notice.
This notice covers website visits and initial enquiries. Therapy records, appointment administration and any agreed WhatsApp support require separate client privacy information before those services begin. Sending this form does not enrol you in WhatsApp support.
